You MUST be able to present a valid, government issued, positive identification card including, but not limited to, a driver's license, passport, or military identification. The identification must contain your photograph and printed date of birth.
A minor may receive a piercing ONLY with a parent/guardian present. Both the minor and parent/guardian must be able to present a valid, government issued, positive identifi
